The Home Care Compliance Checklist: Documents Every Indiana Agency Should Track

The employee documents that come up again and again in Indiana personal-services and home care operations — and how agencies keep them current without a binder.

Ask any Indiana home care administrator what an audit feels like and you'll hear the same story: it's not that the agency did anything wrong — it's that proving it took three days of digging. Compliance is mostly a documents problem, and documents are a tracking problem. Here's the inventory most non-medical home care agencies live and die by.

Per-employee documents that expire

  • TB test results and any required health screenings
  • CPR / first-aid certification cards
  • Driver's license and auto insurance (for staff who transport clients)
  • Professional licenses or certifications where the role requires them
  • Annual performance evaluations and competency checks

Per-employee documents that don't expire (but must exist)

  • Signed application, job description, and offer letter
  • Criminal background check results from hire
  • Signed policy and handbook acknowledgements — for the current version, not the 2021 one
  • Orientation and training records, including annual in-service hours

The three failure modes

Agencies rarely fail because a requirement is unknown. They fail because (1) an expiry passed silently, (2) a policy was updated but nobody re-signed it, or (3) the document exists but can't be produced on the day it's asked for. All three are tracking failures, not knowledge failures — which is why they're so fixable.

What good looks like

One employee file per person, one required-document list per job title, alerts starting 60 days before anything lapses, and the ability to export a complete, current compliance file as a PDF the moment a surveyor emails. That last one turns audit week into an ordinary Tuesday.

One honest caveat: exact requirements vary by license type, county, and payer, and they change. Treat this checklist as the operational baseline and your current state guidance as the source of truth — then make sure whatever the requirement is, your system can prove you met it.
